Students’ applications

You can submit most applications online via USOS system.

1. Log in to the USOS.

2. To send an application, go to the FOR STUDENTS menu and then click on APPLICATIONS.

3. Then select SUBMIT NEW APPLICATION.

4. If you are studying more than one course, select the course you are applying for by clicking on the SELECT AND CONTINUE option in the row for the relevant course.

5. On the next page you will see a list with the types of application available. Once you have read their descriptions and selected the appropriate version of the application, click on COMPLETE APPLICATION.

6. When completing the form, you must select the stage of study to which the application relates and justify your request. You can add a PDF attachment to the application – for most applications, the required attachment will be the transcript of records (*karta przebiegu studiów).
When you have finished filling in the form, click NEXT.

7. You can then choose whether you would like to receive an email confirmation of your application and approve it or withdraw it for editing.

8. Once approved, a message asking you to confirm your application will appear. If you are sure that the application is prepared correctly, click OK, otherwise select CANCEL.

9. When you click OK, you will see that the application has been submitted.

10. The submitted application can be found in the APPLICATIONS module. To check the details of the submitted application, please click on the DETAILS.

11. The response from the Dean of Student Affairs will appear next to the specific application on the list of submitted applications. To read the reasons for the decision, go to the DETAILS tab.

Applications that are not available online must be submitted in print or sent as scans (pdf format) to:
